Exclusive Services
In-home services are provided at patient homes, as above within 10 mile radius from the clinic address without additional cost. In-home services further than a 10 mile radius, but less than 18 miles will incur a $15 travel service fee. In-home visits beyond an 18 miles radius will be billed based on additional travel time to/from patient location and is handled on a case by case basis.
Initial Evaluation
60-75 minutes - $225
Includes written report sent to PCP or MD of choice
Premium visit
60 - 75 minutes - $200
Includes all treatment methods.
Excludes diagnostic ultrasound imaging with report.
Economy visit
(follow up visit)
30 mins - $120
Excludes dry needling, ultrasound guided treatments, or reports.
Rapid Care Visit
60 - 90 mins - $250
Urgent physical therapy evaluation and treatment carried out choice of in clinic or in the patient's home. Includes limited scope diagnostic ultrasound imaging and written report sent to PCP or MD of choice. ​
Telehealth Visit
20 mins - $75
Payment required at time of service, unless other arrangements have been made in advance. Cash and local checks accepted. Credit/Debit card payments to includes 2% processing fee. Returned checks will incur a $35 additional charge.​
